What is carbocisteine used for and how does it work?

Carbocisteine is a medicine known as a mucolytic and it is used to ease some of the symptoms of respiratory conditions such as Chronic Obstructive Pulmonary Disease (COPD) and cystic fibrosis. A chesty cough with phlegm (mucus) is often symptomatic of these conditions and it can be difficult to cough up the phlegm because it is overproduced, thick, and sticky. Carbocisteine helps you to cough up the phlegm by thinning the mucus, breaking it down, and making it less sticky. It’s most effective when taking regularly and patients find it helps them cope with the bothersome symptom of a chesty cough and lots of phlegm that they can’t clear. Although COPD cannot be cured, medicines such as carbocisteine can help ease the symptoms and keep the condition under control.

How do you take carbocisteine?

Carbocisteine comes in capsule form, liquid syrup, or liquid sachet, however, it is most commonly taken in capsule form and you’re unlikely to experience side effects taking it in this way. Always stick to the dosage instructions when taking this medicine, the recommended dose for adults and the elderly is two capsules to be taken three times a day. Once your symptoms improve you may lower your dose to one capsule four times a day.

How long does this medicine take to work and how long should do I take it for?

It may take up to four weeks before you see an improvement in your condition, however, you should continue to take this medicine as it works best when taken regularly. If carbocisteine is working well for you and is keep your symptoms under control, your doctor may recommend that you take this medicine for life. Check with your doctor that it is safe for you to take long term.

Who should not take carbocisteine capsules?

Carbocisteine may not be suitable for people with certain medical conditions. Do not take this medicine if you are allergic to carbocisteine or any of the other ingredients in this medicine. Allergic reactions can be serious and potentially life-threatening so it’s important to avoid this medicine if you’re aware of an allergy.

If you have an ulcer in your stomach or gut you should not take carbocisteine.

Speak to your doctor if any of the following apply to you as it may not be a suitable treatment for you, or you may be more at risk of side effects:

  • You are elderly
  • You have had an ulcer in your stomach or gut in the past
  • You are taking other medicines known to cause bleeding in the stomach

Can you take carbocisteine while pregnant or breastfeeding?

Carbocisteine is not recommended while pregnant, particularly during the first three months of pregnancy as there is not enough information about the safety of this. It is generally safe to take this medicine while breastfeeding, but you should always speak to your GP or pharmacist first.

What are the side effects of carbocisteine?

Carbocisteine capsules don’t usually cause any side effects, however, like all medicines, there is always a possibility of side effects. The most serious side effects that have been reported in rare cases are:

  • Serious allergic reactions - signs include swelling of the face, lips, throat or tongue, difficulty swallowing or breathing, and/or a rash
  • Steven Johnson Syndrome - symptoms include blistering or bleeding of the skin and around the lips, eyes, nose, and genitals, as well as flu-like symptoms and fever
  • Blood in your vomit or black tarry stools

If any of these side effects occur, stop taking this medicine and seek medical help straight away.
