What is Istin?

Istin is a medication which is used to treat high blood pressure or a specific type of chest pain called angina. The tablets are taken on a daily basis when prescribed and can be used by adults and children aged 6 years old and above. When treating angina, Istin does not provide immediate relief of chest pain from the condition, but it can make changes to prevent the symptoms from repeating.

How does Istin work?

Istin contains amlodipine which is classed as a calcium antagonist. What this basically means is that the medication is able to relax blood vessels and allow blood to pass through them more easily. This change naturally reduces the patient’s blood pressure. Also, the heart muscle receives more oxygen, meaning there’s less strain on the chest area.

How do I take Istin?

Istin tablets are available in strengths of 5mg and 10mg. The recommended starting dose is one 5mg tablet a day. The dose may be adjusted to one 10mg tablet a day.

You should aim to take these tablets around the same time each day with a drink of water. The tablets can be taken with or without food. However, avoid taking Istin with grapefruit juice.

When being used by children and adolescents (aged between 6 and 17), the usual recommended starting dose is reduced to 2.5mg a day. 5mg tablets can be divided into halves to provide this particular starting dose. The daily dosage may be increased to 5mg a day later in time depending on the response to treatment.

What are the side effects of Istin?

Like all medicines, Istin can cause side effects but not every patient gets them. The most common side effects reported by users of this treatment include the following:

  • Oedema (fluid retention)
  • Headaches
  • Dizziness
  • Sleepiness
  • Palpitations
  • Flushing
  • Abdominal pain and nausea
  • Altered bowel habits including diarrhoea, constipation and indigestion
  • Tiredness and weakness
  • Visual disturbances and double vision
  • Muscle cramps
  • Ankle swelling

These side effects listed should not last for a long time if they do occur, but you should contact your doctor if any of these symptoms last for more than a week. Some of the rare side effects that can happen while taking Istin can be very serious, so you should seek medical help straight away if you experience any of the following:

  • Sudden wheeziness
  • Swelling around the face, eyelids or lips
  • Swelling of the tongue or throat which causes difficulty breathing
  • A severe skin reaction which could include a rash, hives or reddening of the skin
  • Abnormal heartbeat
  • Inflamed pancreas which could cause severe pain in the abdomen and back

Can I take Istin?

Before taking this medication, it’s important to check that you don’t have any health conditions which would make using Istin unsafe. You should not take this treatment if any of the following applies to you:

  • You have severe low blood pressure (hypotension)
  • You have narrowing of the aortic heart valve (aortic stenosis)
  • You suffer from cardiogenic shock (an issue where the heart can’t supply enough blood to the rest of the body)
  • You suffer from heart failure due to a heart attack in the past

As a precaution, you should inform your doctor or pharmacist if you are elderly, pregnant or breast-feeding. You should also speak to one of them if you have had any of the following conditions:

  • A recent heart attack
  • Severe increase in blood pressure (hypertensive crisis)
  • Heart failure
  • Liver disease

Other medications you are currently taking could possibly clash with the effects of Istin. Inform your doctor or pharmacist about other treatments you are currently taking before you start using Istin, and in particular mention if you are using any of these:

  • Ketoconazole and itraconazole (both anti-fungal medicines)
  • Protease inhibitors for treating HIV (such as ritonavir, indinavir and nelfinavir)
  • Rifampicin, erythromycin and clarithromycin (all antibiotics)
  • Verapamil and diltiazem (both heart medicines)
  • Dantrolene (treats body temperature abnormalities)
  • Medicines that alter how your immune system works (such as tacrolimus and sirolimus)
  • Simvastatin (for lowering cholesterol)
  • Cyclosporine (an immunosuppressant)
  • St. John’s Wort (hypericum perforatum)

What if I take too much Istin?

If you accidentally take a larger dose of Istin than you should, then you should seek immediate medical attention. Taking too many of these tablets could cause your blood pressure to become low or even dangerously low.

Symptoms that could occur include dizziness plus feeling lightheaded and weak. If the drop in your blood pressure is severe enough, shock could occur and you could also lose consciousness. Your skin may also feel cool and clammy.

What if I forget to take Istin?

If you forget to take a dose of this medication, then there’s no need to panic. Just skip the forgotten dose and take the next one at the usual time. It’s unnecessary and risky to take a double dose to make up for a forgotten one.

Usual initial dose is 5 mg ONCE DAILY which may be increased to a maximum dose of 10 mg depending on the individual patient's response on advice of own GP.
