What is Kliovance?

Kliovance is a continuous combined Hormone Replacement Therapy (HRT). It’s taken to reduce the symptoms which can be experienced during the menopause. It contains two types of female hormones which are called oestrogen and progesterone. This particular treatment is a lower dose alternative to other HRT pills and may be used by patients that are more sensitive to hormones compared to others. Kliovance can also be used to prevent the bone-weakening condition osteoporosis, which some women can become more vulnerable to during the menopause.

How does Kliovance work?

This medication contains estradiol and norethisterone, which are artificial versions of the hormones known as oestrogen and progesterone (sometimes referred to as progestagen). The former is used because the amount of oestrogen produced by women significantly drops during the menopause, and it’s this change that’s mainly response for potential symptoms such as hot flushes and headaches.

Since Kliovance is used by women who have not had their womb surgically removed, progesterone is used as a balancing component for the oestrogen. Without this component, women who have not had their womb removed could face greater risk of endometrial cancer when using an oestrogen replacement.

How do I take Kliovance?

A single Kliovance tablet is taken once a day, at about the same time each day. There are 28 tablets in a pack and when they are all taken, a new pack should be started to continue the treatment without interruption. Your doctor or GP will provide guidance on how long you should use HRT for. These tablets can be taken with or without food and drink.

The best time to start using Kliovance depends on whether or not you have been using any other HRT product prior to switching to this one. If this is your first time using HRT, then you should be able to start on any convenient day. But if you are switching from an HRT product which caused you to have monthly bleeding, you should start the treatment once the bleeding has ended.

What are the side effects of Kliovance?

The most commonly reported side effects from taking this medication include tenderness or pain in the breasts and vaginal bleeding. Other potential side effects which are fairly common include the following:

  • Headaches or migraines
  • Weight gain caused by fluid retention
  • Vaginal inflammation
  • Vaginal infection with a fungus
  • Depression
  • Nausea
  • Enlargement or swelling of the breasts
  • Back pain
  • Uterine fibroid (benign tumour)
  • Swelling of the arms and legs

Other possible side effects from using this medication can be very serious, but the worst ones are a rarity and you may ultimately never experience any side effects at all while taking these tablets. It’s always best to talk to your doctor or pharmacist as soon as possible if you do experience side effects, especially if they are causing major concern.

Certain diseases are reported more often in women who use HRT compared to those that do not. These illnesses include the following:

  • Breast cancer
  • Cancer of the lining of the womb
  • Ovarian cancer
  • Blood clots in the veins of the legs or lungs (venous thromboembolism)
  • Heart disease
  • Stroke
  • Probable memory loss if you begin using HRT over the age of 65

Can I use Kliovance?

Kliovance is prescribed to women who have not had surgery to remove the womb and have not had a natural period for at least a year. If you have gone through a procedure to remove your womb already, then you’ll be recommended a different form of HRT which includes oestrogen only.

HRT is not suitable for all women, however, since certain diseases and other medications that may be currently taken can create complications. You should avoid taking Kliovance and talk to your doctor for guidance if you have any of these conditions:

  • You have or have had breast cancer
  • Unexplained vaginal bleeding
  • Excessive thickening of the womb lining that is not being treated
  • You have or have had a blood clot in a vein (thrombosis), such as in the lungs or legs
  • A blood clotting disorder
  • You have or have had a disease caused by blood clots in the arteries (e.g. stroke, heart attack or angina)
  • You have or have had a liver disease and your liver function tests have not returned to normal
  • You have a rare blood problem called porphyria

You should let your doctor know about any medications which you are currently taking or have recently used before you being using Kliovance. In particular, these medications listed can interfere with the effects of this HRT:

  • Medicines for epilepsy (including phenytoin and phenobarbital)
  • Medicines for tuberculosis (including rifampicin and rifabutin)
  • Medicines for HIV infections (including ritonavir and nevirapine)
  • Medicines for hepatitis C infections (including telaprevir)
  • Herbal remedies that contain St. John’s Wort (Hypericum perforatum)
  • Medicines that contain ketoconazole (a fungicide)
  • A concomitant treatment with cyclosporine

It’s also worth checking with your doctor about using this treatment if you have an intolerance to some sugars. That’s because Kliovance contains lactose monohydrate.

What if I use too much Kliovance?

If you use more of this medication than you’re meant to, you should talk to a doctor or pharmacist as soon as you can. Overdosing on this medication can lead to symptoms such as feeling sick and vomiting.

What if I forget to take this medication?

If you forget to take a dose of Kliovance but its less than 12 hours after you were originally due to take it, then take this medication as soon as you remember. If it has already been more than 12 hours since you were meant to take a dose, then skip this forgotten dose and start again as normal on the next day. Never take a double dose to compensate for any forgotten medication.

When a dose of this treatment is forgotten, it is possible that some breakthrough bleeding and spotting could occur if you still have your womb. The likelihood of this occurring increases the more doses that are missed.

Can I use Kliovance just before or after surgery?

If you are due to have surgery, you should let your surgeon know that you are taking this HRT. It’s possible you’ll have to stop taking Kliovance around 4 to 6 weeks before the operation in order to reduce the risk of a blood clot. Check with your doctor when it will be ok to start taking this medication again.
