What is promethazine hydrochloride?

Promethazine hydrochloride is a prescription medication that has many uses. It belongs to a group of medicines known as phenothiazines and it’s used as an antihistamine to relieve hay fever symptoms, as an anti-sickness drug to prevent and treat nausea and vomiting in travel sickness, it’s also a short term treatment for insomnia, and as a sedative. This medicine comes in many forms including tablet, liquid, and injection, for hay fever and allergy symptoms the oral tablet route is used. Promethazine hydrochloride is also sold under the brand name, Phenergan.

What’s the difference between promethazine hydrochloride and Phenergan?

Promethazine hydrochloride is the generic version of Phenergan and this means the only difference between the two drugs is cost, the branded version, Phenergan is usually more expensive. Both of these drugs have the same active ingredient, here the active ingredient is promethazine hydrochloride. Both of these medicines work in exactly the same way, the generic version has the same dosage, strength, effectiveness, safety, and quality as the branded version of the drug.

How do promethazine hydrochloride tablets work?

This medicine is a type of drowsy (sedating) antihistamine as it particularly helps to relieve symptoms of allergies such as urticaria (hives) and pruritus (itching) that is causing difficulty sleeping. Promethazine hydrochloride does this by blocking a chemical that is responsible for allergy symptoms, known as histamine. In an allergic reaction, the body responds by releasing histamine in order to protect you from potentially harmful foreign substances which can cause swelling, redness, itching, etc, but a lot of the time this is an overreaction to substances which shouldn’t be harmful such as pollen and dust. To stop this overreaction, antihistamines block the effects of histamine to prevent and treat the symptoms of an allergic reaction, fast.

To treat nausea, this drug works by blocking another chemical produced in the body known as dopamine.

Who is this medicine suitable for?

This medicine is suitable for adults and children over the age of 2 who suffer from allergy symptoms, however, you should take caution when giving this medicine to children, speak to your GP before doing so.

Promethazine hydrochloride is particularly good for those with hay fever and hay fever that occurs during the night and affects sleep. If you have insomnia that is associated with allergy symptoms such as itching during the night, promethazine hydrochloride may be a suitable treatment for you. This medicine may also be suitable for those who have travel sickness, morning sickness, or vertigo, etc, speak to your GP about using this medicine to treat these conditions.

Who should not take promethazine hydrochloride?

Some people will not be suitable to take this medication, this includes those who have any of the following conditions:

  • Allergy to promethazine hydrochloride or any of the other ingredients in this medicine
  • Should not be given to anyone under the age of 5 years old
  • You are taking monoamine oxidase inhibitors for depression, or have stopped taking this medicine in the last 14 days
  • You suffer from dizziness, drowsiness, headache, or fainting

Speak to your doctor before taking this medicine if you have any of the following:

  • Difficulty breathing, asthma, or bronchitis
  • Epilepsy
  • Serious heart problems
  • Liver or kidney problems
  • Blockage in the stomach or difficulty passing urine
  • Hearing problems
  • Glaucoma
  • Reye’s syndrome or signs of Reye’s syndrome such as being sick or confusion after a viral illness

Speak to your doctor if you’re taking any other medicines before starting promethazine hydrochloride but do not take this medicine if you’re taking monoamine oxidase inhibitors.

Can I take promethazine hydrochloride while I’m pregnant or breastfeeding?

This medicine has been used as a treatment for morning sickness, however, you should speak to your GP before taking this medicine if you’re pregnant. Your doctor will make a decision on whether the benefits outweigh the risks to you and your unborn baby.

Promethazine hydrochloride should be avoided if you’re breastfeeding as there is a possibility that it may pass into the breastmilk and harm your baby.

Precautions to take with promethazine hydrochloride

  • Do not drink alcohol while taking promethazine hydrochloride as it may increase its sedative effects
  • This medicine may affect the accuracy of some test results, particularly pregnancy tests
  • This medicine may cause drowsiness that may be extreme - do not drive or use machinery
  • Promethazine hydrochloride contains lactose and sucrose which is not suitable for people with intolerances to sugar

Promethazine dosage and how to take

The dosage and how you take it depends on the condition it is being used to treat. For hay fever, the recommended dose for adults and children over the age of 10 years is one 25 mg tablet to be taken at night, this may be increased to twice a day.

What are the side effects of Promethazine hydrochloride?

Like all medicines, there are some side effects of promethazine hydrochloride but not everyone will experience them. All of the side effects are listed on the patient information leaflet but if you experience any side effects that aren’t listed in the PIL you should see your GP. If you have any of the following side effects you must stop taking this medicine and seek immediate medical help. These side effects include:

  • Signs of allergic reaction including rash, swelling of the face, lips, tongue, or throat, wheezing or collapsing
  • Liver problems with jaundice
  • Muscle stiffness or shaking, trembling, being unable to control facial muscles, rolling eyes
  • Heart palpitations
  • Irregular heartbeat
  • Neuroleptic malignant syndrome - fever, fast heartbeat, very bad headache, dizziness, muscle cramps/stiffness, confusion, hallucinations, sweating

Speak to your doctor or pharmacist if you have any of the following side effects:

  • Dry mouth
  • Blurred vision
  • Constipation
  • Difficulty passing urine
  • Drowsy, sleepy, tiredness
  • Headache
  • Confusion
  • Nightmares
  • Loss of appetite
  • Nausea or vomiting
  • Diarrhoea
  • Indigestion
  • Stomach upset
  • Skin rash, itching
  • Sensitivity to the sun

How long can you take promethazine hydrochloride for?

This medicine is only intended to be used short term, speak to your doctor if your symptoms are ongoing and you need to take this medicine for a longer period of time. Do not take this medicine for longer than 7 days.
