What is Spiolto Respimat and how does it work?

Spiolto Respimat is a bronchodilator which is a type of medication that makes breathing easier. It is a long-acting bronchodilator which means it is used on a regular basis to manage COPD long term. Bronchodilators help to open the airways and relax the muscles to make breathing easier. This medicine contains two active ingredients, tiotropium, and olodaterol. Tiotropium belongs to a group of medicines called anticholinergics, and it works by blocking the actions of a neurotransmitter known as acetylcholine which can cause involuntary muscle movements in the lungs. Olodaterol is a long-acting beta-2 agonist, and this works by stimulating the muscles in the airways to relax and widen so that air can flow in and out of the lungs. By relaxing the lung muscles, these medicines can improve breathing and reduce episodes of breathlessness.

Can Spiolto Respimat be used to treat asthma?

Although bronchodilators are often used to treat asthma, this type of bronchodilator must not be used if you have asthma. This particular medicine is used to treat COPD only and you should speak to your GP about treatment for your asthma or any other causes of your breathlessness.

Who should not use this medicine?

Some people may be unsuitable to use Spiolto Respimat due to other medical conditions or if there is a risk of interaction between other medicines you may be taking. If you are allergic to tiotropium. olodaterol, or any of the other ingredients in this medicine you should not use this medicine as allergic reactions can be severe and potentially life-threatening. The same applies if you are allergic to atropine or any other substances that are related to ipratropium or oxitropium.

Speak to your healthcare provider if you have any of the following conditions before starting this treatment:

  • Asthma - do not use this medicine to treat asthma
  • High blood pressure
  • Diseases of the heart
  • Epilepsy
  • Thyroid gland problem known as thyrotoxicosis
  • Aneurysm
  • Diabetes
  • Severe liver disease
  • Severe kidney disease
  • Narrow-angle glaucoma
  • Prostate problems or difficulty passing urine
  • You’re due to have surgery

Can I use Spiolto Respimat while pregnant or breastfeeding?

Do not use this medicine if you are pregnant or breastfeeding unless it has been advised by your doctor. Your doctor will weigh up the risks and benefits to you and your baby and will recommend this medicine if it is necessary.

Taking Spiolto Respimat with other medicines

To ensure there isn’t a risk of interaction between Spiolto Respimat and any other medicines, always inform your doctor or pharmacist of any medicines that you’re currently taking whether they’re prescription or non-prescription. This is particularly important if you’re taking any of the following medicines as they may increase the risk of side effects when taken with Spiolto Respimat.

  • Beta blockers
  • Steroids
  • Diuretics
  • Medicines for breathing problems such as theophylline
  • Tricuclic antidepressants or monoamine oxidase inhibitors
  • Any medicines that are similar to Spiolto Respimat and contain similar active ingredients

Spiolto Respimat dosage

This medicine should only be used once a day around the same time each day as the effects last for 24 hours, and the recommended dose is two puffs each time you take it. It’s important that you don’t exceed this dose.

How to use the Spiolto Respimat inhaler

Your inhaler will come with a patient information leaflet which will have step by step instructions on how to use the device. Follow the instructions on how to prepare the inhaler before you use it, if the instructions aren’t clear you can ask your healthcare provider for a demonstration on how to use it.

To use your inhaler firstly turn the clear base halfway in the direction of the arrows while keeping the cap closed. You can then open the cap fully and breathe out slowly while closing your lips around the mouthpiece and being careful not to cover the air vents. Point the inhaler to the back of your throat and take a slow deep breath through your mouth while pressing the dose-release button at the same time. Continue to breathe in slowly for as long as you comfortably can and hold your breath for 10 seconds or as long as is comfortable. Repeat the steps for a total of two puffs and close the cap once you’ve finished.

Spiolto Respimat side effects

Like all medicines there are some side effects associated with Spiolto Respimat but most people won’t experience these side effects. If any of these side effects become bothersome speak to your healthcare practitioner as they may be able to change your treatment or dosage.

The most common side effects associated with this medicine which affect around 1 in 100 people include:

  • Dizziness
  • Headache
  • Fast heartbeat
  • Cough
  • Hoarseness
  • Dry mouth

In rarer cases (1 in 1,000 people) you may experience a sore throat, inflammation of the larynx, gums, or mouth, fungal infections of the mouth and throat, changes in heartbeat such as rapid heartbeat or palpitations, difficulty sleeping, nausea, joint pain, infections of the urinary tract, and other side effects that are listed on the patient information leaflet.

Look out for the following serious side effects and if any of these occur, stop using the medicine immediately and seek urgent medical help. Serious side effects include:

  • Immediate allergic reaction after using the inhaler which causes symptoms such as a rash, swelling of the mouth and face, difficulty breathing, low blood pressure, and lightheadedness
  • Tightness in the chest, coughing, wheezing, or breathlessness
  • Seeing halos around lights or coloured images
  • Blockage in the intestines or lack of or absence of bowel movements
